Dog Surrender Request Form

HSSW is a managed intake facility, which for us means that we keep a close eye on our population (the number of animals we are caring for) and bring in more pets as we are able to based on that population. Because of this, we almost always work off of a waitlist for surrendering pets and schedule surrender appointments based off of the movement of animals in and out of our shelter.
 
While we try to help any animal that needs to be surrendered, there are circumstances that the animal may not be a good fit at our facility. If we cannot take your pet in, we will try to provide any other resources we may have to help assist you.
 
Please also be aware that filing out this request form does not guarantee that we will be able to schedule a surrender appointment with you. It is simply the start of the surrender process and a greater conversation of how we can best help you and your dog.
