Welcome and thank you for adopting a pet from the Humane Society of Southern Wisconsin! Please note that you will not receive an approval or denial based solely on the information provided via the form below. We instead take a conversations-based approach to adoption and focus on matchmaking pets with interested adopters.

To complete this application, you must:

Would you like to pay an additional $25.00 for FeLV Antigen and FIV Antibody Testing?
Would you like to pay an additional $36.00 for an FeLV Vaccine?
This vaccine will need to be boosted in 3-4 weeks. We can schedule this booster with our Clinic or Intake Teams. Call us the week your pet needs their booster to schedule this. *NOTE: Your cat does need to have a negative FeLV/FIV test prior to receiving the vaccine.
